Coordinated Family Support (CFS)
Supporting individuals. Strengthening families. Planning together.
Coordinated Family Support helps adults with developmental disabilities who choose to live in their family home. Available to adults age 18 and older who receive regional center services, CFS helps individuals and families coordinate resources, address challenges, and build the supports needed for a stable home life.
We proudly serve Fresno and Merced counties and the Oakhurst area, providing compassionate, dependable care that helps each person thrive.

What support can include
Service coordination
organizing and scheduling services so supports work together
Skills at home
coordinating opportunities to develop skills that support independence in the family home
Caregiver support
arranging training so the people providing support better understand the individual’s needs
Transportation
identifying transportation options for appointments, services, and community activities
Accessing resources
identifying additional services and helping families understand how to access them
Community connections
connecting individuals with activities, resources, and self-advocacy groups
Backup planning
identifying alternative providers when usual supports are unavailable
Future planning
preparing for changes in family circumstances and long-term support needs
Services are selected with the individual and family to address their priorities and the barriers they face.
Support centered on the individual and family
CFS respects the individual’s choices, family relationships, language, and culture. The individual and family help shape a personalized plan that reflects their goals, preferences, and home environment.
Planning for changing needs
As caregivers age or circumstances change, CFS can help families prepare for future care arrangements and transitions. It complements the regional center service coordinator’s role without replacing or duplicating that support.
Getting started
Adults and families interested in CFS should contact their regional center service coordinator. Through an Individual Program Plan (IPP) team meeting, they can discuss whether CFS would help. Following a referral, the CFS provider works with the individual and family to develop a plan outlining their goals and the assistance needed.
CLIENT RIGHTS — Coordinated Family Support (CFS)

Primary focus: Supporting adults who choose to remain in their family home.
Clients have the right to individualized support coordination that respects their personal choices, family relationships, language, and culture.
Clients should be supported in:
participating in decisions about their care and services.
accessing medical appointments and community resources.
receiving support in their preferred language.
having their cultural and family preferences respected.
developing greater independence while living with family.
planning for future needs and changes in caregivers.
identifying backup support when regular caregivers are unavailable.
Important: CFS should support the adult client's wishes and autonomy while coordinating with family members. Family involvement does not automatically override the adult's decisions.
